Nurse Practitioner - OH, MO, WI, IN Licensed

Job Summary

Providing telephonic care, the part-time Nurse Practitioner - OH, MO, WI, IN Licensed will manage patient assessments and care coordination while working remotely with a commitment of 20 hours per week, including evenings, nights, and rotating weekends.

Key responsibilities:

• Deliver telephonic and telehealth care, coordinating with caregivers, facilities, and providers
• Perform comprehensive assessments and document findings in compliance with regulations
• Coordinate care transitions and monitor patient needs to adjust care plans as necessary

Required qualifications:

• Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
• Active and unrestricted Nurse Practitioner license in Ohio, Missouri, Indiana, or Wisconsin
• Board certified through a national board in Family, Adult, Gerontology, or Adult-Gerontology Acute Care Nursing
• Current DEA licensure/prescriptive authority or ability to obtain post-hire
• Proficient in electronic medical records and documentation practices
