Nurse Practitioner - PA and NY Licensed

Job Summary

A company is looking for a Nurse Practitioner for the National After-Hours Team, licensed in PA and NY.

Key Responsibilities

• Provide telephonic support and medical care to residents and facility staff after-hours
• Coordinate care using a telephonic platform, managing calls between facilities, hospitals, and clinics
• Deliver comprehensive assessments and ensure compliance with CMS requirements while documenting encounters in the EMR

Required Qualifications

• Master's Degree as a Nurse Practitioner
• Current unrestricted NP license in Pennsylvania, New York, and/or Michigan
• Nurse Practitioner national certification as ANP, FNP, or GNP
• 3+ years of experience as a Nurse Practitioner
• Proficient computer skills for documenting medical information in EMR
