Nurse Practitioner - PA NY MI Licensed

Job Summary

A company is looking for a Nurse Practitioner for the National After-Hours Team, part-time, licensed in PA, NY, or MI.

Key Responsibilities

• Provide telephonic medical care and support to residents and facility staff after-hours
• Coordinate care using a telephonic platform, managing calls between facilities, hospitals, and clinics
• Conduct comprehensive assessments and document encounters in the EMR, ensuring compliance with CMS requirements

Required Qualifications

• Master's Degree as a Nurse Practitioner
• Current unrestricted NP license in Pennsylvania and New York and/or Michigan
• Nurse Practitioner national certification as ANP, FNP, or GNP
• 3+ years of experience as a Nurse Practitioner
• Ability to work scheduled shifts in accordance with scheduling policies
