Key Responsibilities

• Supervise patient monitoring and coordinate treatment regimens for chronic conditions
• Conduct monthly audits to ensure clinical quality standards and implement performance enhancement strategies
• Provide guidance and mentorship to team members, fostering a collaborative environment

Required Qualifications

• 5+ years of Nurse Practitioner experience treating chronic diseases
• Master's Degree as a Nurse Practitioner with a willingness to expand state licensure
• 5+ years of experience leading a clinical team
• Board certification and active license required (ANCC/AANP)
• Multi-state compact RN licensure
