Job Summary

A company is looking for a Nurse Practitioner for its National After-Hours Team, licensed in TX, LA, and TN, on a part-time basis.

Key Responsibilities

• Provide telephonic support and medical care for residents and facility staff after-hours
• Coordinate care between facilities, hospitals, and clinics using a telephonic platform
• Deliver comprehensive assessments and document encounters in the EMR while ensuring compliance with CMS requirements

Required Qualifications

• Master's Degree as a Nurse Practitioner
• Active, unrestricted licensure in Texas and Louisiana, or Tennessee
• Nurse Practitioner national certification as ANP, FNP, or GNP
• 3+ years of experience as a Nurse Practitioner
• Background in acute and chronic disease management
