NY Licensed Social Worker

Job Summary

To support comprehensive care management for members with chronic and complex illnesses, the remote NY Licensed Social Worker will manage care plans, coordinate services across various settings, and facilitate member education and behavioral health strategies.

Key responsibilities

• Develop and communicate individualized care plans in collaboration with members, caregivers, and healthcare providers
• Conduct assessments and provide care management to meet psychosocial and health care needs throughout the continuum of care
• Evaluate members' Social Determinants of Health and facilitate access to community resources and mental health support

Required qualifications

• Bachelor's degree required; Master's in Social Work (LMSW) is mandatory
• Certification in clinical social work (LCSW) preferred
• 4 - 6+ years of case management and/or managed care experience
• Knowledge of Federal and State regulations for Medicare and Medicaid
• Proven experience in leading care management teams and health plan operations
