Job Summary

A company is looking for a Care Management Support Coordinator III.

Key Responsibilities
  • Perform outreach to members to assist with care plans, resources, and scheduling
  • Serve as front-line support for member and provider inquiries, explaining care plan procedures
  • Document and maintain non-clinical member records in compliance with state and regulatory requirements
Required Qualifications
  • High School diploma or GED required
  • 2 - 4 years of related experience required
  • In-depth knowledge of care management support activities preferred
  • Experience with member onboarding and administrative duties
  • Ability to support training of new hires may be required
