Job Summary
Supporting the Care Coordination department remotely, the Case Management Associate will manage inbound inquiries, process referrals, confirm Medicaid eligibility, and conduct Health Risk Assessments while maintaining professionalism and adaptability in a fast-paced environment.
Key responsibilities
- Provide administrative support by managing the inbound care management queue and responding to inquiries from members and guardians
- Process referrals for the program, confirming Medicaid eligibility and completing Health Risk Assessments for assigned members
- Adapt to changing operational needs and maintain accuracy while working in complex data systems and electronic record systems
Required qualifications
- Must reside in Ohio
- Experience in administrative support within a healthcare or case management setting
- Familiarity with Medicaid processes and health risk assessments
- Ability to work independently while maintaining communication with team members
- Knowledge of HIPAA regulations and commitment to maintaining confidentiality
