Job Summary
To enhance patient care, the full-time remote Ohio Licensed Ambulatory Care Pharmacist will provide Comprehensive Medication Management (CMM), collaborate with healthcare teams, and develop individualized therapeutic regimens for chronic disease management.
Key responsibilities:
- Provide Comprehensive Medication Management (CMM) in collaboration with clinical care teams
- Develop medication management action plans to achieve member self-management goals
- Communicate with internal and external providers to align clinical interventions with medication-related goals
Required qualifications:
- PGY1 pharmacy practice residency and PGY-2 Ambulatory Care Pharmacy Residency, or
- Minimum of 2 years' experience as a clinical pharmacist in an ambulatory care setting
- Active Ohio pharmacist license
- Experience in developing clinical pathways and protocols
- Ability to conduct medication assessments and initiate or adjust medications per protocol
