Ohio Licensed Ambulatory Care Pharmacist

Job Summary

To enhance patient care, the full-time remote Ohio Licensed Ambulatory Care Pharmacist will provide Comprehensive Medication Management (CMM), collaborate with healthcare teams, and develop individualized therapeutic regimens for chronic disease management.

Key responsibilities:

• Provide Comprehensive Medication Management (CMM) in collaboration with clinical care teams
• Develop medication management action plans to achieve member self-management goals
• Communicate with internal and external providers to align clinical interventions with medication-related goals

Required qualifications:

• PGY1 pharmacy practice residency and PGY-2 Ambulatory Care Pharmacy Residency, or
• Minimum of 2 years' experience as a clinical pharmacist in an ambulatory care setting
• Active Ohio pharmacist license
• Experience in developing clinical pathways and protocols
• Ability to conduct medication assessments and initiate or adjust medications per protocol
