Job Summary

A company is looking for an Ambulatory Social Worker - Population Health.

Key Responsibilities

• Provide psychosocial assessments and develop comprehensive treatment plans for patients and families
• Participate in interdisciplinary treatment teams and provide consultative services to healthcare team members
• Facilitate therapy sessions, document interactions, and maintain electronic medical records according to guidelines

Required Qualifications

• Master's Degree in Social Work or related field
• Relevant licensure as a social worker in the applicable state
• Two years of social work experience
• Certification in Case Management is preferred
• Ability to obtain Master's-level licensure in Ohio within 6 months of hire
