Job Summary

A company is looking for a Care Coordination Supervisor.

Key Responsibilities

• Manage and supervise the Care Coordination team, ensuring adherence to High Fidelity Wraparound Principles
• Provide leadership, training, and support to team members while ensuring service coverage and crisis support
• Participate in community meetings and maintain client records, ensuring compliance with all documentation requirements

Required Qualifications

• At least 3 years of experience in children's behavioral health or related fields, or 2 years with a Bachelor's degree, or 1 year with a Master's degree
• Master's degree and license in a behavioral health field preferred
• Experience in family systems, case management, or child protection
• Must have a valid driver's license and acceptable driving record
• CPR/First Aid and NCI/CPI Certifications required within 90 days of employment
