Job Summary
To support youth with complex needs and their families, the full-time Ohio Licensed Care Coordinator will engage clients, conduct assessments, and develop individualized care plans while working remotely and in community settings.
Key responsibilities
- Effectively engage children/youth with significant behavioral health needs and their families for an assigned caseload
- Conduct comprehensive CANS assessments and facilitate the development of individualized, strength-based care plans
- Monitor service delivery and facilitate Child and Family Team meetings to ensure the child and family’s voice is included in their care plan
Required qualifications
- At least 3 years of experience in community-based services for children and youth, or 2 years with an Associate/Bachelor's degree, or 1 year with a Master's degree
- Experience in areas such as family systems, community resources, case management, or child protection
- Training in high-fidelity wraparound and ability to manage complex cases
- Valid driver's license with an acceptable driving record and insurability through the employer
- CPR/First Aid and NCI/CPI certifications required within 90 days of employment
