Job Summary

A company is looking for a Care Coordinator in Internal Medicine.

Key Responsibilities

• Collaborate with a multidisciplinary care team to develop goals and plan interventions for high-risk patients
• Provide coordination of care and disease management for patients with chronic conditions
• Conduct comprehensive clinical assessments and monitor patient compliance with care plans

Required Qualifications

• Graduate from an accredited school of Professional Nursing
• Current state licensure as a Registered Nurse (RN)
• Basic Life Support (BLS) Certification through the American Heart Association (AHA)
• Three to five years of recent nursing experience
• Background in adult care, med/surg, pulmonary, inpatient, or ambulatory settings
