Job Summary

A company is looking for a Care Coordinator for Union County.

Key Responsibilities

• Engage children/youth with significant behavioral health needs and their families for assigned caseload
• Conduct comprehensive assessments and develop individualized care plans
• Facilitate access to necessary resources and monitor service delivery to ensure compliance with care plans

Required Qualifications

• At least 3 years of experience in community-based services for children and youth, or equivalent education and experience
• Experience in family systems, case management, or child protection
• Culturally competent with experience managing complex cases
• Must complete high-fidelity wraparound training within specified time
• Valid Driver's license and acceptable driving record required
