Ohio Licensed Care Coordinator

Job Summary

A company is looking for a Care Coordinator for youth with complex needs and their families.

Key Responsibilities

• Engage children/youth with significant behavioral health needs and their families for assigned caseloads
• Conduct comprehensive assessments and develop individualized care plans
• Facilitate access to clinical consultations and ensure ongoing monitoring of services

Required Qualifications

• At least 3 years of experience in community-based services for children and youth, or relevant education with reduced experience
• Experience in family systems, case management, or child development
• Culturally competent with experience managing complex cases
• Valid Driver's license and acceptable driving record
• CPR/First Aid and NCI/CPI Certifications required within 90 days of employment
