Job Summary

A company is looking for an EHP Care Coordinator.

Key Responsibilities

• Manage a caseload of approximately 700+ members in chronic condition programs through health coaching and care coordination
• Collaborate with a multidisciplinary team to assess member health and document interactions via electronic platforms
• Guide members in managing chronic conditions and support them in meeting program requirements for wellness benefits

Required Qualifications

• Graduate from an accredited school of nursing or behavioral health science
• Current state licensure as a Registered Nurse (RN), Licensed Professional Clinical Counselor (LPCC), or Licensed Independent Social Worker (LISW)
• Three years of clinical experience
• Knowledge of chronic condition programs and federal/state HIPAA regulations
• Strong analytical and computer skills
