Ohio Licensed Care Coordinator

Job Summary

A company is looking for a Care Coordinator, Advanced (IKC).

Key Responsibilities

• Coordinate and facilitate quality, cost-effective care for ESRD and CKD patients
• Collaborate with healthcare team members to minimize fragmentation in care delivery
• Support high-risk IKC patients to reduce hospital readmissions through intensive care coordination

Required Qualifications, Training, and Education

• A.D.N degree from an accredited school of nursing required; B.S.N preferred
• Current Registered Nurse (RN) license in the state of practice required
• Minimum of five (5) years' experience in clinical nursing required
• Minimum of three (3) years' experience in renal nursing preferred
• Current CPR certification required
