Job Summary
To support youth with complex needs and their families, the full-time Ohio Licensed Care Coordinator will engage with clients, conduct assessments, and develop individualized care plans while working remotely in Crawford or Wyandot County.
Key responsibilities
- Effectively engage children/youth with significant behavioral health needs and their families for an assigned caseload
- Conduct comprehensive CANS assessments and facilitate the development of individualized, strength-based care plans
- Facilitate Child and Family Team meetings, ensuring the child and family have a voice in their care and access to necessary supports
Required qualifications
- At least 3 years of experience in community-based services for children and youth, or 2 years with an Associate/Bachelor's degree, or 1 year with a Master's degree
- Experience in areas such as family systems, case management, or child development
- Culturally competent with the ability to manage complex cases involving children and families experiencing SED and trauma
- Must complete high-fidelity wraparound training within a specified timeframe
- Valid driver's license and acceptable driving record required
