Job Summary
Seeking an Ohio Licensed Care Coordinator, the full-time remote position will manage comprehensive care coordination for youth with complex needs and their families, focusing on engagement, resource linkage, and individualized care planning.
Key responsibilities:
- Engage children and youth with significant behavioral health needs and their families for an assigned caseload
- Conduct comprehensive assessments and develop individualized Child and Family-Centered Care Plans
- Facilitate Child and Family Team meetings to ensure the family's voice is included in care planning
Required qualifications:
- At least 3 years of experience in community-based services for children and youth, or equivalent education/experience
- Background in family systems, case management, or related fields
- Culturally competent with experience managing complex cases involving SED and trauma
- Valid Driver's license and acceptable driving record
- CPR/First Aid and NCI/CPI Certifications required within 90 days of employment
