Job Summary
To support youth with complex needs and their families, the full-time Ohio Licensed Care Coordinator will engage clients, conduct assessments, and develop individualized care plans while working remotely in the community.
Key responsibilities
- Effectively engage children/youth with significant behavioral health needs and their families for an assigned caseload
- Conduct comprehensive CANS assessments and facilitate the development of individualized, strength-based Child and Family-Centered Care Plans
- Facilitate Child and Family Team meetings, ensuring the child and family have a voice in their care and access to necessary resources
Required qualifications
- At least 3 years of experience in community-based services for children and youth, or relevant education and experience combinations
- Background in family systems, community resources, case management, or related fields
- Culturally competent with experience managing complex cases involving children and families
- Completion of high-fidelity wraparound training program recognized by ODM
- Valid driver's license with an acceptable driving record and insurability through the organization's vehicle insurance carrier
