Job Summary
To support youth with complex needs and their families, the full-time Ohio Licensed Care Coordinator will engage clients, conduct assessments, and develop individualized care plans while working remotely within the community.
Key responsibilities
- Effectively engage children/youth with significant behavioral health needs and their families for an assigned caseload
- Conduct comprehensive assessments and facilitate the development of individualized care plans
- Monitor service delivery and facilitate Child and Family Team meetings to ensure client involvement in their care
Required qualifications
- Minimum of 3 years of experience in community-based services for children and youth, or relevant education with reduced experience
- Background in family systems, case management, or child protection
- Culturally competent with experience managing complex cases involving trauma and behavioral health disorders
- Valid Driver's license and acceptable driving record
- Completion of CPR/First Aid and NCI/CPI Certifications within 90 days of employment
