Job Summary
To support youth with complex needs and their families, the full-time Ohio Licensed Care Coordinator will manage comprehensive care coordination activities remotely while engaging with community resources and facilitating individualized care plans.
Key responsibilities
- Effectively engage children/youth with significant behavioral health needs and their families for assigned caseloads
- Conduct comprehensive CANS assessments and facilitate the development of individualized, strength-based care plans
- Facilitate Child and Family Team meetings, ensuring the child and family have a voice in their care and access to necessary supports
Required qualifications
- At least 3 years of experience in community-based services for children and youth, or relevant education and experience combination
- Background in family systems, case management, or child and family counseling
- Culturally competent with experience managing complex cases involving SED, trauma, and co-occurring disorders
- Must complete high-fidelity wraparound training within a specified timeframe
- Valid driver's license and acceptable driving record required
