Ohio Licensed Care Management Associate

Job Summary

Supporting Ohio Medicaid behavioral health care coordination, the full-time Care Management Associate will manage inbound queues, process referrals, verify Medicaid eligibility, and complete Health Risk Assessments while working remotely from home in Ohio.

Key responsibilities

• Manage inbound care management queue and respond to member/guardian/provider inquiries
• Take and process referrals; confirm Medicaid eligibility and document accurately
• Complete Health Risk Assessments (HRA) for assigned members

Required qualifications

• Ohio resident required (remote position)
• High school diploma or GED
• 1+ year relevant experience, including high call volume work
• Strong computer skills and accuracy across multiple systems, including Excel
• Dedicated, private, distraction-free workspace for remote work
