Ohio Licensed Care Management Coordinator

Job Summary

A company is looking for a Care Management Coordinator to support dual eligible populations in a remote setting.

Key Responsibilities

• Conduct annual Health Risk Surveys to identify member needs
• Inform care managers of newly identified health/safety risks or service needs
• Complete care coordination activities as delegated by the care manager

Required Qualifications

• 2+ years in behavioral health, social services, or a related field
• Proficient in Microsoft Office Suite
• Access to a private, dedicated workspace
• High School Diploma or equivalent experience required
• Associate's or Bachelor's Degree in behavioral health or human services preferred
