Ohio Licensed Care Manager

Job Summary

A company is looking for a Care Manager - Transitions of Care.

Key Responsibilities

• Engage members and their support systems through strength-based assessments and trauma-informed care
• Develop and update individualized care plans in collaboration with the inter-disciplinary care team
• Coordinate care and facilitate communication among members, providers, and community organizations

Required Qualifications, Training, and Education

• Nursing degree from an accredited program or Bachelor's degree in a healthcare field, or equivalent experience
• Preferred advanced degree associated with clinical licensure
• Minimum of three years of experience in nursing, social work, counseling, or related healthcare profession
• Preferred three years of experience in Medicaid and/or Medicare managed care
• Current unrestricted clinical license as a Registered Nurse, Social Worker, or Professional Clinical Counselor required
