Job Summary

A company is looking for a Care Manager, Registered Nurse.

Key Responsibilities

• Engage with members telephonically to coordinate their care and develop individualized care plans
• Provide disease management education and connect members with resources to support their health goals
• Document care management activities accurately and collaborate with the interdisciplinary care team

Required Qualifications

• Active and unrestricted Registered Nurse (RN) licensure in the state of OH or compact licensure
• Proficient in Microsoft Office Suite and capable of utilizing healthcare technology tools
• Minimum of 3+ years of nursing experience
• Minimum of 2+ years of case management or home healthcare coordination experience
• Access to a private space for remote work and ability to work independently
