Job Summary

A company is looking for a Care Manager RN - Work From Home.

Key Responsibilities

• Engage with members telephonically to coordinate their care and develop individualized care plans
• Provide disease management education and connect members with health plan benefits and community resources
• Document care management activities accurately and collaborate with the interdisciplinary care team to address barriers to care

Required Qualifications

• Active and unrestricted Registered Nurse (RN) licensure in the state of residence or compact licensure
• Proficient in Microsoft Office Suite
• Minimum 3+ years of nursing experience
• Minimum 2+ years of case management, discharge planning, or home healthcare coordination experience
• Access to a private workspace for remote work
