Required Qualifications

• Active and unrestricted Registered Nurse (RN) licensure in Ohio or compact licensure
• Proficient in Microsoft Office Suite
• 3+ years of nursing experience
• 2+ years of case management, discharge planning, or home healthcare coordination experience
• Access to a private workspace for remote work
