Job Summary
To support patient care coordination, the full-time Ohio Licensed Clinical Intake Specialist will manage the clinical review and documentation process during patient intake, ensuring accurate information is relayed to field-based clinicians while working remotely.
Key responsibilities:
- Review referral packets and document patient diagnoses and clinical summaries in the EMR
- Ensure compliance with ICD-10 coding guidelines and address documentation gaps with referring providers
- Identify and escalate trends in documentation quality to leadership for resolution
Required qualifications:
- A current Ohio Licensed Practical Nurse (LPN) or Physical Therapist Assistant (PTA) certification is required
- Minimum of 2 years' experience in the health care industry, with a preference for home health intake experience
- Medical coding experience is required
- Knowledge of clinical best practices, HIPAA regulations, and home health agency guidelines is essential
- Experience with ICD-10 coding and Medicare Home Health documentation requirements is preferred
