Ohio Licensed Clinical Intervention Pharmacist

Job Summary

A company is looking for a Clinical Intervention Pharmacist-Medicare Part D Stars.

Key Responsibilities:

• Conduct targeted member and/or provider clinical interventions to ensure appropriate and cost-effective member drug therapy
• Develop strategies to engage members and/or providers and drive improvements in pharmaceutical care and pharmacy quality measures
• Review drug/medical literature, clinical practice guidelines, quality measures, and national guidelines to support drug therapy interventions

Required Qualifications:

• Doctor of Pharmacy (Pharm.D.) or Bachelor of Pharmacy with relevant experience
• 3 years of pharmacy practice experience in a managed care pharmacy setting
• 2 years of medication therapy management, quality, or similar managed care pharmacy experience
• Current unrestricted license from the Ohio State Board of Pharmacy
• Comprehensive understanding of managed care pharmacy, drug utilization management, population health, and quality programs
