Ohio Licensed Clinical Nurse Navigator

Job Summary

Providing comprehensive care coordination, the full-time Ohio Licensed Clinical Nurse Navigator will deliver virtual clinical support, manage patient care plans, and facilitate communication among healthcare teams in a hybrid work environment.

Key responsibilities

• Orchestrate comprehensive care delivery models leveraging technology and nursing expertise for remote patient monitoring and case management
• Provide clinical triage and support through established protocols during patient communications
• Collaborate with stakeholders to develop and implement individualized care plans while promoting a positive work environment

Required qualifications

• BSN (Bachelor's Degree in Nursing) required
• Ohio Registered Nurse (RN) license required upon hire
• 3+ years of acute care hospital in-patient nursing experience required
• Experience with care management plans and knowledge of Medicare and Medicaid regulations preferred
• Strong organizational skills and proficiency in telephone triage preferred
