Job Summary
Collaborating with an interdisciplinary clinical team, the part-time Licensed Clinical Social Worker (LCSW) will provide comprehensive, patient-centered palliative care remotely, focusing on psychosocial support, care coordination, and serious illness conversations.
Key responsibilities
- Conduct comprehensive assessments to evaluate psychosocial distress and identify priority needs for patients with serious illness
- Facilitate serious illness conversations and advance care planning with patients and families, ensuring alignment with patient goals
- Deliver clinical recommendations to the care team and provide short-term counseling and emotional support to patients and caregivers
Required qualifications
- Master's in Social Work (MSW) from an accredited program
- Active, unrestricted Licensed Clinical Social Worker (LCSW) license in Ohio
- Clinical experience in palliative care or hospice, particularly with advance care planning
- Experience working within an interdisciplinary care model
- Proficiency in using electronic health records (EHRs) and clinical documentation systems
