Job Summary

To revolutionize healthcare delivery, the full-time Ohio Licensed Family Nurse Practitioner will conduct virtual wellness visits, health assessments, and provide comprehensive care coordination while working remotely across various locations in Ohio.

Key responsibilities

• Conduct thorough health risk assessments and medication reviews during virtual wellness visits
• Collaborate with in-home clinical team members to evaluate patient needs and arrange necessary services
• Provide comprehensive care coordination, including preventive care interventions and referrals to specialists

Required qualifications

• Board Certified as a Family Nurse Practitioner
• Active Family Nurse Practitioner license in Ohio
• Credentialed Medicaid provider for the state of Ohio
• Demonstrated professionalism, integrity, and compassion in healthcare delivery
• Technologically savvy with experience using mobile devices for patient care coordination
