Job Summary

Seeking an Ohio Licensed Family Nurse Practitioner, the full-time position will conduct virtual wellness visits, health assessments, and provide comprehensive patient care while working remotely.

Key responsibilities

• Conduct thorough health risk assessments and medication reviews during virtual wellness visits
• Collaborate with in-home team members to evaluate vital signs and coordinate patient care
• Utilize technology for patient management and documentation while maintaining high-quality care standards

Required qualifications

• Board Certified as a Family Nurse Practitioner
• Active Family Nurse Practitioner License in Ohio
• Must be actively registered with Medicaid
• Demonstrated professionalism and compassion in patient care
• Technologically savvy with experience using mobile devices for charting and communication
