Ohio Licensed Independent Social Worker

Job Summary

Collaborating with an interdisciplinary clinical team, the remote Licensed Independent Social Worker (Ohio) will deliver comprehensive palliative care, addressing the psychosocial and emotional needs of patients with serious illnesses while facilitating care coordination and advance care planning.

Key responsibilities

• Conduct comprehensive assessments to evaluate psychosocial distress, mental health needs, and social determinants of health
• Facilitate serious illness conversations and advance care planning with patients and families
• Provide clinical recommendations and support care coordination within the interdisciplinary team

Required qualifications

• Master's in Social Work (MSW) from an accredited program
• Active, unrestricted Licensed Clinical Social Worker (LISW) license in Ohio
• Clinical experience in palliative care or hospice, including advance care planning
• Experience working within an interdisciplinary care model
• Proficiency in using electronic health records (EHRs) and clinical documentation systems
