Job Summary

A company is looking for a Chronic Kidney Disease Advanced Practice Provider (NP or PA).

Key Responsibilities

Deliver direct nephrology care to CKD patients, ensuring medication optimization and effective planning for dialysis initiation
Collaborate with the nephrology care team and coordinate holistic care with family members and caregivers
Participate in quality improvement initiatives and maintain accurate patient records in compliance with legal guidelines

Required Qualifications, Training, and Education

Master's or Doctoral degree in Nursing with a specialization in nephrology care
Active RN and APRN license in at least one of the specified states
Minimum of 2 years of experience in a nephrology practice or dialysis unit
Proficiency in telehealth technology and electronic health records (EHR)
Bilingual in English and Spanish is preferred
