Job Summary

A company is looking for a Chronic Kidney Disease Advanced Practice Provider (NP or PA).

Key Responsibilities
  • Deliver direct nephrology care to CKD patients, ensuring medication optimization and effective planning for dialysis initiation
  • Collaborate with the nephrology care team and coordinate holistic care with family members and caregivers
  • Participate in quality improvement initiatives and maintain accurate patient records in compliance with legal guidelines
Required Qualifications, Training, and Education
  • Master's or Doctoral degree in Nursing with a specialization in nephrology care
  • Active RN and APRN license in at least one of the specified states
  • Minimum of 2 years of experience in a nephrology practice or dialysis unit
  • Proficiency in telehealth technology and electronic health records (EHR)
  • Bilingual in English and Spanish is preferred
