Job Summary

A company is looking for a Registered Nurse Care Manager (RN).

Key Responsibilities

• Assess patient clinical, functional, and behavioral health status for timely follow-up and service allocation
• Coordinate with scheduling, authorization, and billing teams to ensure documentation integrity and resource utilization
• Oversee the agency care management program, ensuring high-quality and cost-effective care delivery

Required Qualifications

• Registered Nurse license in the state of employment
• Two years of home care field experience preferred
• Minimum of one year of clinical review experience preferred
• Two years of case management and/or utilization management experience preferred
• Proficient in HCHB
