Job Summary

A company is looking for a Nurse Practitioner (NP) to conduct remote health risk assessments in Cleveland, OH and surrounding areas.

Key Responsibilities

• Conduct comprehensive in-home health risk assessments to identify active and chronic disease conditions
• Provide written documentation of patient visits in accordance with NCQA standards
• Communicate with patients, caregivers, and other healthcare providers to ensure proper diagnosis and care

Required Qualifications

• Active/unrestricted nurse practitioner license in coverage states
• Board certification from ANCC, AANP, or NCCPA
• Active BLS Certification
• Current enrollment in Medicare/Medicaid
• Valid driver's license with a good driving record
