Job Summary
To support high-quality palliative care delivery, the remote Ohio Licensed Palliative Care Provider will lead clinical care teams, coordinate with network providers, and analyze population-level data to enhance patient outcomes.
Key responsibilities
- Lead internal and joint case rounds to ensure effective care plan development and execution
- Coordinate services with Network and Longitudinal Care Providers for optimal patient support
- Monitor and analyze network data to identify trends and implement necessary interventions
Required qualifications
- Master of Science in Nursing (MSN) or Master of Science in Physician Assistant studies
- Five years of clinical experience in palliative care or hospice care
- Active Registered Nurse or Physician Assistant license in Ohio without board actions
- Board certification as an advanced practice nurse by a nationally recognized certifying agency
- Active Unrestricted DEA license
