Job Summary

A company is looking for an Outpatient LPN for the Patient Advice and Care Team.

Key Responsibilities

• Perform initial and ongoing health status assessments and document findings
• Document planned nursing interventions and coordinate care plans based on patient needs
• Evaluate and document responses to nursing interventions and participate in process improvement activities

Required Qualifications

• High School diploma or GED
• Current LPN license from the State of Ohio
• Graduate from an accredited school or program of practical nursing
• BLS - Basic Life Support certification from the American Heart Association
• Successfully completed Ohio Board of Nursing approved medication course
