Ohio Licensed Practical Nurse
Job Summary
To support a centralized Medication Refill Call Center, the full-time Ohio Licensed Practical Nurse will provide nursing care, perform health assessments, and collaborate with the healthcare team while working Monday through Friday from 9:00 a.m. to 5:30 p.m.
Key Responsibilities
- Conducts initial and ongoing health status assessments and documents findings to inform care plans
- Documents and coordinates nursing interventions based on patient needs and collaborates with the healthcare team
- Participates in process improvement activities and acts as a patient advocate to enhance clinical quality and customer satisfaction
Required Qualifications
- High School diploma or GED
- BLS - Basic Life Support certification from the American Heart Association
- LPN - Licensed Practical Nurse certification from the National Council of State Boards of Nursing
- Preferred 3-5 years of LPN experience
- Preferred minimum of 1 year of Primary Care experience
