Ohio Licensed Practical Nurse Care Manager

Job Summary

A company is looking for an LPN Care Manager to work from home in the Columbus area.

Key Responsibilities

• Perform initial and ongoing health status assessments, documenting subjective and objective data
• Document planned nursing interventions and coordinate care plans to achieve patient outcomes
• Evaluate responses to nursing interventions and revise care plans as necessary

Required Qualifications

• High School diploma or GED required
• Current LPN license from the State of Ohio
• Completion of an Ohio Board of Nursing approved medication course
• BLS - Basic Life Support certification from the American Heart Association
• 1-3 years of nursing experience preferred
