Job Summary

To support patient transitions of care, the part-time PRN Ohio Licensed Registered Nurse Liaison will manage patient healthcare coordination services, attending discharge rounds and ensuring eligibility for home care while working remotely after initial training in the Akron office.

Key Responsibilities

• Identifies future patients and determines home care eligibility
• Coordinates healthcare services as ordered by the attending physician
• Builds and maintains relationships with patients, facility personnel, and physicians

Required Qualifications

• Associate degree with a minimum of two years' experience or a combination thereof
• Licensed in the state of services as a Registered Nurse (RN)
• Experience in patient healthcare coordination and discharge planning
