Ohio Licensed Resource Nurse

Job Summary

Providing nursing support to members with low-acuity, short-term clinical needs, the full-time Ohio Licensed Resource Nurse will conduct focused transitions of care assessments, deliver clinical education, and perform in-person clinical exams, all in a 100% remote setting.

Key Responsibilities

• Conduct outreach to members during inpatient admissions and post-discharge for transitions of care assessments
• Perform comprehensive medication reconciliation and address quality gaps through proper documentation and coding
• Review charts and clinical data to identify members needing higher levels of care management or short-term interventions

Required Qualifications

• Graduate of an accredited school of nursing (R.N.)
• 3+ years of nursing experience
• Strong critical thinking skills with sound clinical judgment
• Proficiency in using technology for remote member engagement
• Ability to apply Motivational Interviewing and Trauma-Informed Care principles
