Ohio Licensed RN Admissions Coordinator

Job Summary

A company is looking for a Remote RN Admissions Coordinator Long-Term Care & Rehabilitation.

Key Responsibilities:

• Coordinate admissions and transitions of care for members entering long-term care or rehabilitation facilities
• Review clinical documentation to determine appropriate facility placement based on member needs
• Serve as the liaison between health plans, facility admissions teams, and providers to ensure timely placements

Qualifications:

• Active, unrestricted RN license in the State of Ohio
• At least 3 years of clinical experience in home health, senior living, or long-term care admissions
• Strong knowledge of Medicaid systems, authorizations, and coverage criteria
• Proficiency with electronic medical records (EMR) and care coordination tools
• Ability to work independently in a high-volume, fast-paced environment
