Ohio Licensed RN Care Coordinator

Job Summary

Supporting high- and rising-risk patients, the part-time Nightingale RN Care Coordinator will leverage clinical expertise for comprehensive care coordination and chronic disease management while working remotely between 8:00 AM and 4:30 PM.

Key responsibilities

• Conduct patient outreach and develop patient-centered care plans to improve health outcomes
• Collaborate with multidisciplinary teams to reduce preventable emergency department visits and enhance patient satisfaction
• Provide mentorship and support to new Care Coordinators, ensuring compliance with regulatory standards

Required qualifications

• Graduate from an accredited school of professional nursing
• Current state licensure as Registered Nurse (RN)
• Basic Life Support (BLS) certification through American Heart Association (AHA) or American Red Cross
• Three years of experience in a Care Coordinator position at the Cleveland Clinic
• Current demonstrated clinical competence
