Job Summary

A company is looking for an RN Care Manager to oversee patient care and coordination.

Key Responsibilities

• Develop and oversee patient plans of care, ensuring effective communication between field staff and patients
• Assess patient clinical, functional, and behavioral health status for timely service allocation and management
• Coordinate interdisciplinary team meetings and support field nurses with physician communication

Required Qualifications

• Registered Nurse license in the state of employment
• Two years of home care field experience preferred
• Minimum of one year of clinical review experience preferred
• Two years of case management and/or utilization management experience preferred
• Proficiency in HCHB and capable of meeting all physical demands
