Job Summary

A company is looking for an RN Care Manager.

Key Responsibilities

• Develop and oversee patient plans of care, ensuring accuracy and regulatory compliance
• Participate in interdisciplinary team meetings and manage healthcare costs through effective care planning
• Assess patient clinical, functional, and behavioral health status for timely follow-up and service allocation

Required Qualifications

• Registered Nurse licensure required
• Two years of home care field experience preferred
• One year of clinical review experience preferred
• Two years of case management and/or utilization management experience preferred
• Knowledge of Medicare, Medicaid, and compliance guidelines for patient care
