Job Summary

Key Responsibilities

Systematically collects and assesses comprehensive health data for patients
Develops and implements patient-centered care plans to achieve expected outcomes
Evaluates patient progress and communicates findings to the healthcare team

Required Qualifications, Training, and Education

Bachelor of Science in Nursing or Master's Degree in Nursing or a related field preferred
Minimum of 3 years of clinical nursing experience required
Basic Life Support (BLS) certification from the American Heart Association required
Valid, active Registered Nurse (RN) license in the state of Ohio required
Certification by a nationally recognized care management organization preferred
