Ohio Licensed RN Care Manager

Job Summary

To support patient care continuity, the full-time Ohio Licensed RN Care Manager will oversee the development and management of patient care plans, collaborate with field staff, and participate in interdisciplinary team meetings while working remotely.

Key responsibilities:

• Review and revise patient Plans of Care (POC) to ensure accuracy and compliance with regulatory standards
• Participate in interdisciplinary team meetings and agency townhall meetings to enhance collaboration and care planning
• Monitor healthcare costs and ensure adherence to utilization management guidelines while managing patient clinical assessments

Required qualifications:

• Registered Nurse licensure required
• Two years of home care field experience preferred
• One year of clinical review experience preferred
• Two years of case management and/or utilization management experience preferred
• Proficiency in HCHB and knowledge of Medicare, Medicaid, and compliance guidelines preferred
